Are offers based on account (ie. Google Play) or by phone?

Seeing posts saying that in order to "redo" an offer, you need to factory reset the phone or use a different phone. However, wouldn't first time user imply the specific Google Play account never connected with the game? Also, are there concerns with getting banned for this repeat methodology?
