Weird thing happened during my lube IHUT

I’m not sure how to handle this one. I got an IHUT for a new kind of lube. I received the sample, and the sample came with a piece of paper detailing when the surveys would come out.

Now I’m on SB a lot, all hours of the day, every day.

I only received the first survey. I answered it without issue.

I never received another survey. I’m not sure how to follow up, or if there’s anything I can do at all.
